OpenAI product lead Tibo has responded to reports of GPT-5.6 accidentally deleting files, confirming that the most common cause was the model setting its temporary directory to $HOME, then mistakenly deleting the user's home directory instead. The company says these incidents mostly occurred under Full Access mode with Codex's sandbox and automatic review disabled. The team is updating developer prompts, steering users toward safer permission modes, and adding additional safeguards. OpenAI calls the incidents "extremely rare" and promises a full postmortem in the coming days.
